Reporting for L-1 ID Solutions Security Breach
If you were contacted by L-1 Identity Solutions in reference to a security breach
and you believe your identity might have been stolen, you can report suspicious
activity on your credit history by completing the following statement and affidavit
form.
Instructions:
- Please complete all requested information, if available.
- If a report has already been made with your local law enforcement agency, please
include the information on page 5 of the affidavit portion.
- If you have not filed a report with any other agency, disregard page 5 of the affidavit.
- You should not need to contact any other agency.
- An investigator will be contacting you at the phone number you provide to supply
you with a case number and any other information you may require.
