Submitting Checks

In order to submit checks to our office, you will need to fill out the information form we provide, and attach a legal copy* of the check (see below for a further explanation of acceptable legal copies).

It is preferable, but not mandatory, to have the Check Submission Form (PDF) notarized. We can do this free of charge for you in our office.

We send any legal notices necessary, and victims are not required to send notice to hot check writers. However, if you wish to do so, an example of an appropriate demand letter is provided below.

*The “legal copy” we need will usually be the check 21 that most banks or institutions will provide to you. View an Example Demand Letter (PDF).
Cannot Provide the Check 21
If you cannot provide the check 21, your submitted copy needs the following:
  • To be certified by the financial institution rejecting it
  • A stamp indicating the reason for return
  • A date of rejection
